Direct Support Staff

Enjoy the friendship of our wonderful consumers who have developmental disabilities! We are looking for reliable, patient, and fun people who, in addition to supporting our consumers’ daily living, can interact with them, be their role models, and motivate them to work on their goals. This person also needs to be open-minded, organized, and can take on unexpected challenges. Work locations in San Jose and Fremont.

Qualifications:

•Has High School diploma or GED, undergraduate degree preferred, and at least one year of experience working with developmentally disabled individuals or an enthusiastic, quick leaner with impeccable work ethics
• Clear finger printing criminal record, in good health and clear TB test record
• Current certification in CPR and First Aid
• Good living habits, no smoking, pass drug testing
• Prefer person own a safe car, valid California driver’s license & good driving record
• Has ability to represent FCSN in a professional manner at all times
• Is fluent in oral and written English
• Is able to get along with co-workers, Case Managers, participants and their families
• Is able to attend monthly staff meetings, trainings, and supervision
• Respects the cultural and linguistic values of the participants, their families and their Circle of Support
• Bi-lingual (in Spanish, Mandarin, or American Sign Language) candidates are highly encouraged to apply
 

Specific Job Description and Responsibilities:

• Establishes and maintains a supportive relationship with participants
• Implements independent living skill curriculum in the areas of communication, self-advocacy, money management, mobility trainings, developing practical social, leisure and recreational skills, community integration, preventative and on-going health maintenance, etc.
• Provides services in the location that is best for learning whether it be the home of the participant or the community
• Submits complete and accurate monthly participant updates to Case Manager in a timely manner
• Respects the privacy of participants and maintains confidentiality of information obtained in the course of professional services
• Follows appropriate training or support schedules as agreed upon with individual participants and the Case Manager
• Minimizes health, safety and any other risks to the well being of participants
• Accompanies participants to appointments, as appropriate and requested
• Fosters maximum self-determination on the part of participants

 

Respite Worker

FCSN Respite Program is currently looking for a patient, detail-oriented and dependable worker to provide respite care for the children/adults with developmental disabilities who reside with their parents or family members to ensure their safety and self-care needs met.

Job duties:

Provide appropriate care, supervision and support to protect the consumer’s safety.
•Assist in attending basic self-help needs.
•Provides intermittent or regularly scheduled temporary non-medical care and supervision at the consumer’s own home .

Hours:

Flexible schedule (seven days a week, hours are flexible, minimum of 4-hour respite service each time) to meet family needs.
