Direct Support Staff

We are looking for reliable, patient, and fun people who, in addition to supporting our consumers’ daily living, can interact with them, be their role models, and motivate them to work on their goals. This person also needs to be open-minded, organized, and can take on unexpected challenges. Work locations in San Jose and Fremont.

Qualifications:
Specific Job Description and Responsibilities:
• Establishes and maintains a supportive relationship with participants
• Implements independent living skill curriculum in the areas of communication, self-advocacy, money management, mobility trainings, developing practical social, leisure and recreational skills, community integration, preventative and on-going health maintenance, etc.
• Provides services in the location that is best for learning whether it be the home of the participant or the community
• Submits complete and accurate monthly participant updates to Case Manager in a timely manner
• Respects the privacy of participants and maintains confidentiality of information obtained in the course of professional services
• Follows appropriate training or support schedules as agreed upon with individual participants and the Case Manager
• Minimizes health, safety and any other risks to the well being of participants
• Accompanies participants to appointments, as appropriate and requested
• Fosters maximum self-determination on the part of participants
Respite Worker
FCSN Respite Program is currently looking for a patient, detail-oriented and dependable worker to provide respite care for the children/adults with developmental disabilities who reside with their parents or family members to ensure their safety and self-care needs met.

Job duties:
Hours:
•Flexible schedule (seven days a week, hours are flexible, minimum of 4-hour respite service each time) to meet family needs.
