We provide

Supported Living Services (SLS) -

Supported Living Service is a lifestyle option for adult with disabilities. The individual lives in a home of his/her choice, with or without roommates, and with support from friends, family and staff, to have a life that is typical of others in the community. The supports are intended to remain in place for as long as necessary, perhaps forever.

- Individual Living Service (ILS) -

           Individual Living Service is a way for adults with disabilities to live on their own. There are 2 levels of ILS:

  •      While still in the family home, training can be provided to help the person learn skills necessary to live away from home. This is generally limited to no more than 2 years.
  •      Once an individual is living on their own, a limited numbers of hours of assistance with the complicated issues of living can be provided.

 

 

 

Eligibility

The adult with disabilities who wishes to receive ILS service must be a Regional Center consumer. The initial application for service must be made through the Regional Center case manager and be approved by the Regional Center for funding.

Language & Culture

FCSN was developed to serve families from Chinese cultures. It has expanded to include a recognition and competence in a variety of cultures. Besides working in English, we specialize in Mandarin and Cantonese. Another area of expertise is American Sign Language (ASL).
